Davis Evaluation—Symptom profile/perceptual assessment                                                 $200
Purpose: Testing to see if Davis Program is an appropriate program for client
(Required for all Davis programs)

Reading Evaluation                                                              
Purpose: To evaluate the client’s reading struggles
Testing for:

1. Reading level
2. Comprehension,
3. Use of cue systems for reading,
4. Dyslexic issues

Dyslexia Evaluation
Purpose: To see if the client struggles with dyslexic tendencies: omissions, repetition, or substitutions of sight words, disorientation  

Writing (Dysgraphia) Evaluation
Purpose: To evaluate a client’s writing 
Testing for: proper formation of letters, spatial awareness, disorientation, line or motion triggers,   sentence structure, turning or movement of letters

Reports Include:

• Detailed Report (Ekwall/Shaker Informal Inventory)                                                   
• Complete Reading/Cue System/Grade Level Evaluation
• Current or future school accommodations
• Recommendation for 504 plan
• Outline of Program

In the past, parents have requested reports documenting the child’s progress. Schools would not make accommodations without the reports. For this reason, I have made this service available. Payment is due at the time of program. (Sample is available upon request.)

*If both the TGM assessment and the Davis assessment are requested, there will be no charge for the Davis assessment.
